Purpose:
This questionnaire helps you examine where you currently get your sense of worth, what strengthens it, and what may cause you to surrender it to other people.
How to complete it
For each statement, score yourself:
1 = Never true
2 = Rarely true
3 = Sometimes true
4 = Often true
5 = Consistently true

PART 1 — KNOWING YOUR WORTH
